Terms & Policies
Reservation Policies:
After a referral is made and verified, the client will be contacted to schedule an initial assessment within 24 hours. The Kinesiologist will provide a report with program recommendations based on the initial assessment. SHIFT Rehab Services provides direct billing after the payee information has been verified and sessions have been approved. Further sessions can then be scheduled or reservations can be made by contacting the SHIFT Rehab Services Office here.
Cancellation Policy:
Please provide 24 hours notice of cancellation. We ask that you provide as much notice as possible so we can either reschedule your session or fill your previous session. If adequate notice is given no cancellation fees will apply. Late cancellations and no-show appointments will be billed to the payee or client directly.
Check-in:
Please arrive 15 minutes early for your scheduled session. Be prepared and in the appropriate attire for your service (ie. swim attire for hydrotherapy, running shoes and gym attire for the gym or home exercise program, etc.)
Facilities:
SHIFT Rehab Services works with you, your rehabilitation team, and your location to choose the best facility to meet the goals of the program and needs of the client. The facility can be your home, a community recreational facility, or outdoors in a location closest to your residence. Our goal is to make it easy and accessible for you to participate fully in your treatment program, without causing more stress on your day. We come to you.
Medical Concerns:
During the initial assessment, the client will complete the necessary exercise prescription and medical history forms to ensure safety and effectiveness, moving forward with your treatment plan. A doctors approval may be requested by SHIFT Rehab Services to ensure all safety measures have been met. Our Kinesiologists follow the guidelines set forth by the BCAK Scope of Practice and work with other health care providers to ensure client safety at all times.
Privacy Policy
The privacy of your personal information is important to our clinic. We are committed to collecting, using and disclosing personal information responsibly and only to the extent necessary for the goods and services we provide.
Like all medical professionals, we collect, use and disclose personal information in order to serve our patients. The primary purpose for collecting personal information is to provide treatment.
